A lever-arch file for every month
A client books anti-wrinkle injections and some filler on the same day. That means two consent forms, plus your general terms, signed on a clipboard in the chair while the numbing cream sets. The forms go into a folder for the month, in whatever order they were signed.
Six months later, the client comes back and asks about a previous treatment. You go looking for the consent form. Which month was it? Was it the old version of the filler form, before you added the new section? Is it in the folder at all, or did it get left in the treatment room?
Meanwhile you have updated your toxin consent form twice this year, and you are not sure the old copies in the drawer have all gone.
Why consent paperwork gets away from you
Aesthetics consent is treatment-specific. A clinic offering toxin, filler, skin boosters, peels, microneedling and laser can have a different form for each, and they get revised as practice and insurers' expectations change.
- One form per treatment type, sometimes several per visit.
- Forms revised over time, with old printed copies still in use.
- Signing done in the chair, rushed, just before treatment.
- Paper filed by date, not by client or treatment.
- Consent to photos and marketing mixed into the same form or missing altogether.
Your insurer and your own policy decide what the forms must say. The problem here is making sure the right form is signed for every treatment, and that you can find it again.

If a complaint or insurance claim ever arises, the first thing anyone asks for is the signed consent for that treatment on that date. Being able to produce it in seconds matters.
Digital consent per treatment
- Your consent forms rebuilt as digital forms, one per treatment, each with a version number.
- When a treatment is booked, the matching form is sent automatically, so the client can read it at home before the day.
- Anything unsigned by the appointment can be completed on a clinic tablet in reception, not in the chair.
- Signature, date, time and form version are stored against the specific treatment record in your clinic software.
- Photo and marketing consent captured as separate choices, not buried in the treatment form.
- When you revise a form, the old version stops being sent, and returning clients are asked to sign the new one at their next treatment of that type.

For clinics with a lot of paper history, we can scan and index older consent forms by client and treatment, so they are findable even if they will never be digital.
A clinic where consent is never a hunt
Clients arrive having already read and signed, so the chair is for the treatment. Every treatment record shows its consent, signed on the correct version. When a client asks about a treatment two years ago, the form is on screen before they finish the question.
The cabinet of monthly folders can be retired, or at least stops growing.
